A Message From The Town Clerk On The Dog License Application

 

It has come to my attention that the dog license application form that went out with Annual Street List mailing does not have the fees and other important information listed.  Attached here is the correct form. 

Missing info:

In order to license your dog(s), you will need the following:

• A copy of a valid rabies certificate
• License fee: Spayed/Neutered $6  /  Not Spayed/Neutered $10
• Checks payable to “Town of Ashby”

If licensing by MAIL you will also need the following:

• Dog Licensing Form only required for mail in registrations
• Self-Addressed stamped envelope to have tags mailed to you
